Implementation Approaches

Focus: Alcohol and drug use in our community

  • Provide training to EMS agencies and quarterly continuing education
  • Screen for alcohol use as a cause for falls and injuries presented in our hospital ED when indicated
  • Refer ED patients to drug and alcohol programs when indicated

  • Provide medical oversight and training to law enforcement for Narcan administration
  • Refer ED patients to drug and alcohol programs when indicated
  • Support and actively engage in local drug Take-Back programs and promote availability of prescription drug collection boxes throughout the county

  • Provide appropriate follow-up with non-emergent patients using the ED for primary care
  • Ensure coverage and care for uninsured and Medicaid-eligible patients using our ED for primary and dental care by actively screening patients for coverage through the Marketplace or financial assistance programs and assisting with application processes
  • Refer patients as appropriate to Health Care Network
  • Accept vouchers from Health Care Network for eligible diagnostic lab and radiology services
  • Provide Behavioral Health coordinator in our ED to assess and appropriately expedite referrals for behavioral health services

Focus: Senior care: chronic disease and fall prevention

  • Assure that patients and frail elderly persons referred to our Senior Resource Nurses are assessed for:
    - Behavioral health needs
    - Cognitive impairment (using MoCA -- Montreal Cognitive Assessment)
    - Family and social support
    - Home visits when possible
    - Nutrition needs
    - Pharmaceutical assistance program
    - Safe-At-Home or appropriate supportive placements
    - Transportation needs for follow-up care
  • Provide patient education and support for:
    - Advance Directives (Power of Attorney)
    - Family guidance and support services
    - Medication management/safety
  • Provide community education programs, Stepping On and StrongBodies

Focus: High blood pressure and obesity and the link to chronic disease; cancer

  • Offer free screenings for blood pressure and Body Mass Index (BMI) in a variety of community settings and at community events
  • Provide educational information on blood pressure, cholesterol and overweight/obesity
  • Provide free drop-in blood pressure screenings at AHCMG clinics

  • Hepatitis C: Patients in cohort are identified and screened; Referrals, staging and standardized care delivery process for our patient population are implemented for those who test positive

Focus: Sexual Assault

  • Provide trauma-informed and victim-sensitive services to people of all ages who have been affected, including forensic evidence collection, through 24/7 coverage
  • Provide SDFI®-TeleMedicine forensic photodocumentation based on and designed around the Federal Rules of Evidence
  • Refer as appropriate to medical, clinical, counseling and advocacy services

Focus: Newborn health and safety

  • Provide two babysitting classes annually to area teens, with education on babysitting as a business, baby development, baby safety with CPR and choking, baby care, and including a video by the American Safety & Health Institute
