Implementation Approaches

-Increase community awareness of Mental Health and Alcohol and Other Drug Abuse (AODA) problems and collaborate for better case management and navigation of treatment

  • Collaboration with Federally Qualified Health Centers (FQHC’s) behavioral health strategies/offerings

-Expand health resources to assist, support, and navigate through community based clinical services and insurance coverage

  • Utilize Community Health Worker to improve readmissions and /or navigation for high risk chronic conditions for patients in 53208 zip code
  • Actively participate in Milwaukee Health Care Partnership
  • Provide paths to improve health insurance access to underserved populations in Milwaukee County

  • Explore partnership with food outlets to promote healthy food choices in Milwaukee County and those serving the Washington Park community (53208, 53206, 53204 zip codes)
  • Explore opportunities for expanding community health education /resources (Faith-Based communities, local FQHC’s, Milwaukee Co. health departments)

-Implement programs to increase awareness, screenings and early detection at Froedtert Hospital and within Milwaukee county

  • Provide quarterly cancer awareness and community education events
  • Dedicated Community Outreach Coordinator to work with underserved populations in Milwaukee County by providing community resources, navigation and education
  • Execute a minimum of two community cancer screenings per year
  • Partnership with the MCW’s cancer team
  • Introduce new service of mammography and ultrasound at Progressive Community Health Center

Violence

-Collaborate with community partners across sectors to inform programming being brought to the Washington Park neighborhood.

  • Partner with Milwaukee County crime prevention programs (i.e. Milwaukee Homicide Review Board)
  • Explore research based programs ( i.e. Cardiff Model in the ED program to reduce street violence and accident and emergency (A & E) violence related attendances)
  • Explore partnership with non- profit organizations to support reductions in violence
