Implementation Approaches

Goal: Ensure all pregnant women in Dane County have the opportunity for a healthy birth regardless of race, ethnicity or socioeconomic factors.

Initial Objective: Eliminate disparities in low birth weight births between African American and White women in Dane County

Tactics: 

  • Tobacco Cessation: Reduce smoking cessation rates by optimizing tobacco cessation screening, counseling and referral in primary care and Ob-Gyn

  • Centering Pregnancy Group Prenatal Care: Provide prenatal care, health education and social support by expanding CenteringPregnancy to a third site in collaboration with community partner organization

  • Centering Pregnancy Group Prenatal Care: Provide prenatal care, health education and social support by expanding CenteringPregnancy to a third site in collaboration with community partner organization
  • Address Bias within Healthcare: Reduce bias within healthcare by implementing computer-based training and facilitated discussions under direction of Chief Diversity Officer
  • Social Determinants of Health: Identify social determinants of health by implementing standard screening questions and facilitating referrals to community resources
