Implementation Approaches

Need: Substance Abuse

Initiatives:

1. Assist in the establishment of a regional AODA community recovery/treatment center.

Need: Chronic Disease & Management

Initiatives: 

2. Utilize Gundersen Moundview’s website to improve community access to physical activity programs including “Minutes in Motion” and local health and wellness opportunities.

3. Participate in community health initiatives including the Adams County Cancer Awareness Team and Be Healthy Adams County coalitions and related programming.

Need: Substance Abuse

Initiatives:

1. Assist in the establishment of a regional AODA community recovery/treatment center.

2. Participate in the Drug Free Adams County Coalition and related programming.

3. Develop and implement standardized opioid protocols in the emergency department and primary care clinic.

4. Improve the process for individualized pain contracts in primary and specialty care clinics.

Need: Health Care Cost & Access

Initiatives:

1. Increase affordable healthcare by implementing a broadened sliding scale charity care policy.

2. Improve access to primary and specialty care providers in cardiology, endocrinology and pediatrics through in-house providers or telemedicine.

3. Develop alternative delivery methods, such as telemedicine, to improve stroke, neonatal and pediatric care and outcomes.

4. Determine the feasibility of establishing a federally qualified, sliding-fee dental clinic.

5. Improve access to mental health by developing a collaborative arrangement with area health care providers and/or regional and state-wide partnerships.

6. Implement a process for depression screening for annual wellness or preventive care visits.

7. Develop a protocol to establish consistent primary care treatment of behavioral health issues. Pilot the behavioral health protocol with one primary provider.

8. Participate on a state-wide mental health committee exploring primary care treatment.

9. Sponsor and participate in community programs to educate the public on mental health.

1. Assess and screen primary care patients on preventive care with a focus on blood pressure, diabetes and cancer.

2. Utilize Gundersen Moundview’s website to improve community access to physical activity programs including “Minutes in Motion” and local health and wellness opportunities.

3. Participate in community health initiatives including the Adams County Cancer Awareness Team and Be Healthy Adams County coalitions and related programming.

Need: Workforce Development

Initiatives: 

1. Implement Gundersen Moundview staff development strategies by means of offerings of certifications, continuing education/workshops and tuition reimbursement programs.

2. Provide job shadowing opportunities to high school and college students.

3. Provide clinical rotations and internship opportunities to future healthcare providers.
